Lack of focus and discipline would be at the top of the list. Too short term oriented is at the top, same thing as lack of focus and discipline I suppose. Probably a lot more and these might not be the worst. Keep jumping around with different approaches. But, I think it all boils down to focus and discipline. Which probably comes from a deep inherent lack of belief and/or success with any one process. Getting better, I think. Best guesses.
This is not uncommon. But there is no getting better without rules.
